LifeSynch is continuously looking for qualified professional behavioral healthcare providers. We encourage all qualified providers to apply.

Solo or Group Practice

Please note: If you are practicing in a group (same tax ID number), your group must submit an application or have one on file.

A group is one or more providers filing under the same tax ID number.

Please note: An individual application is also required for each provider in the group.

LifeSynch uses this online service to help physicians and other healthcare providers with the credentialing process.
