For agreements executed prior to June 1, 2011 the manual is effective Sept.1, 2011 and objections to the revised provider manual must be in writing and sent to LifeSynch, Attn: Provider Relations, 2101 W. John Carpenter Freeway, Irving, Texas 75063 by Sept. 1, 2011.
